HSE observation round & action register
HSE supervision round producing an OPEN observation register (not a fixed checklist): during the visit, the inspector records both good practices to recognise and the unsafe acts and conditions to correct — each with risk category, severity (High = non-conformity), location, agreed action (recognition or corrective), deadline and photo evidence (repeatable section). Includes a formal close-out and review loop: weekly summary, contractor response/actions, review status (A→E) and sign-offs — for managing actions on a multi-stakeholder project (client, supervision/PMCM, contractor). Supporting the requirements of ISO 45001 (6.1.2 hazard identification, 8.1.2 risk reduction, 10.2 corrective action). To be distinguished from: fixed-point compliance checklists (Weekly inspection NP-CHA-002, QSE visit NP-CHA-001); BBS focused on operator behaviours (NP-OBS-001); and a priori risk assessment by task (HIRA NP-CHA-004, start-of-shift NP-CHA-003). Here the observation is free, after the fact, with action tracking and contractor close-out.
